I heard a story of a business owner who got fed up with his staff always wondering "Who's to blame?".
He wrote a note and put it in his desk. Whenever the blame game started he'd pull the note out to show everyone.
It said:
"It's my fault."
Then he'd say, "Now, how can we fix it and prevent it happening again?"
