As someone who avoids crappy food at all costs, I too had this thought.

Chipotle uses organic ingredients and a good portion of the animal proteins are organic or pasture-raised

Panera is a close second

As for a more fast-food approach and not fast-casual, Amy's the frozen organic food line had plans to create McDonalds style food establishments with good food - i think only on the west coast for now

We have something like this in Toronto called Flock. Its a fast rotiseree(spelling?) Chicken and salad place. They use a lot of stuff like rye kernels, wakame, goji berries, etc. Its taken off rather well, expanding to 3 locations in the past 2 years and I believe are looking to franchise. The food is really good, and very health conscious.

To expand on this, I just found out they are adding an additional 3 locations in Toronto. Demand for this is definitely there.

Every chipotle I've been in has been way faster than McDonald's.

Personally I love the idea of a healthy fast food place. Price needs to be max $7 for a meal. I think the soup idea is worthy. Easy to make a huge quantity to keep costs low. I'm not too concerned with being able to consume while driving.

i got a suspicion that soup has gotta be one of the healthiest most ancient ways to prepare food.
u literally throw a bunch of shit into cold water and just heat it up. Since the water is cold, all the magical vitamins, minerals, etc flow out of the ingredients into the water.
Take bone for example, how many people consume bone broth or bone type food, that's like some kind of superfood isn't it? but nowadays we just eat the meat of an animal and call it a day.

I love making soup, except for the washing of the pot afterwards 3 days later.

I don't know about the United States but here in Holland we have a healthy food chain which is doing great. They opened in 2015 and have multiple chains across the country already.

I guess this exact concept could be copied in the US. I would be surprised if such a thing doesn't exist in the states yet since almost all health trends are initiated in the US.

This is the chain I'm talking about: SLA - ORGANIC SALAD BAR
