How often do you revise your plan?

I made a 4-year place about a year ago. I was on track until a slight derailment a few months ago, but I'm still making progress and going in the right direction. My plan definitely needs revision though to account for the new aspects of my life.

I'm wondering if I've taken too long to revise my plan... How often do all of you revise your plan and keep it up-to-date? How often do you check it to see if you are on track?
