How do you format your books?

Hi,
I've been using people on fiverr to format my books but kind of want to do it my self now.

I have tried just loading up the word file or using mobipocket converter to get a html file. I have had mixed results but usually it looks a bit funky with the ipad (in the previewer). I've searched around on google but was wondering how others format their books here. Do you just upload the word file? Do you do something to it to make it look good in all devices etc?

It would be good to hear how others do it.
