How do you go about selling advertising directly, that is, without the use of a hosted platform such as Google AdSense.

Assuming you have traffic numbers, and contacts lined up:

- How do you accept payment? Schedule recurring payments with PayPal or?...

- How do you go about tracking statistics for running ads (clicks, impressions, etc.)?

- Has anyone used DoubleClick for Publishers (formerly Google AdManager)? Do you recommend it?

- Since more and more people are using AdBlock+ or similar alternatives, how do you go about beating those plugins?

I think you may be over thinking this.

Throw up an "Advertising" page on your site, directing queries to you, and perhaps link to a press kit (which simply details the site's demographics/readership numbers)

Put up a banner ad on the site, with the text "Advertise Here".

See what happens. If nothing, then take another step. Contact businesses that might be interested in being in front of your audience, and offer them advertising space. See what happens. If nothing, take another step.....

I think you may be over thinking this.

He isn't if he has a lot of traffic (meaning a few million ad views a month).

I use DFP on my site. I like that it is free, you can setup parameters to only show some ads to certain geo regions, limit number of times a user sees an ad and all that stuff. There isn't that much support from Google other than the help screens. I have found a good consultant that can explain the complexities and helped me set some things up. It will handle all the reports you are talking about, but not the billing.
