These are just my personal opinions.
Hosting - Hostgator, bluehost (price varies depending on if you buy it in packages or not)
Domain name - name cheap, yahoo (around $10)
Shopping cart - paypal (free to set up, but I think you may have to pay a transaction fee as with any merchant/shopping cart)
Website designer - no idea, you can find some simple templates and a free site making program called NVU
Copywriter - elancer (varies with the person)
Press relase writer - Write them your self
Ghostwriter - elancer (varies with the person)
It seems to me that you are not going to write any of your ebook since you would like a ghost writer. I will tell you that hiring someone to write a whole ebook is very expensive and probably not worth it.
Most, if not everyone who has their own ebook writes it themselves. You are selling people your product which means you should have your ideas in the book and not some ghost writer.
If you need anything else let me know.
