Should I sell a service first and then get licensing?

I'd like to provide a service to doctors offices, but I have not spoken to the company who offer these service about licensing agreement. Should I attempt to negotiate an agreement first and promise them clients? Or should I obtain some clients and promise the service for them? I am currently working on the numbers to present to both parties.
